Community Health Nurses (CHN) are responsible for planning services for individuals, families, and the community. In order to effectively plan, it is essential that CHN assess the resources specific to the community a client resides in. A windshield survey is an assessment method utilized to obtain a visual overview of the community in order to note conditions and trends that could affect the health of the population. With data from this assessment, the CHN gains insight into the background and context of the community, identifies health-related resources available, and begins to understand gaps in services. The CHN may also use data from the assessment to collaborate with the individual, family, or community and develop interventions to improve health and wellness.

Step 2: Select a geographic community around one of the hospitals where you have worked. (GEOGRAPHIC COMMUNITY TO USE IS WILTON MANOR, FLORIDA https://www.wiltonmanors.gov/)
Step 3: Make plans to complete your windshield survey. You can complete the survey by driving around in your car. If possible, it would be easier to ask a family member/friend to drive while you survey the community. I would also recommend walking around in your chosen community, visiting and learning about community organizations and resources, taking pictures of your chosen community to share with us, or interviewing a community resident. Additionally, you may use Google Earth as another tool for completing the windshield survey.
Step 4: Summarize the findings of your windshield survey using the following template (ALSO ATTACHED BELOW): Windshield Survey Template. The template may be completed with bullet-point formatting. Then, copy and paste the table to the initial discussion board post along with answers to the following supplemental questions:
a) Describe the physical and mental health status of the community including any effects COVID19 has had on the physical and mental health of the population. Use one of the following websites to add to your information:

b) What do you perceive as the biggest need in this community? What evidence led you to this conclusion?

d) Choose a caring theory, e.g., Boykin & Schoenhofer’s Nursing as Caring, Leininger’s Theory of Culture Care, Watson’s Theory of Human Caring, Roach’s 6 C’s, et cetera. How would you incorporate this caring theory into the health education project?
